New worker’s compensation patients coming to California Occupational Medical Professionals (COMP) need a DWC 1 form (https://www.dir.ca.gov/dwc/DWCForm1.pdf) or a written authorization to treat from their claims carrier -this can be emailed to comprfa@gmail.com.

New patients coming in for new Worker’s compensation injury, DOT physical or pre-employment physical will be seen on the same day.
